Health Exchange is a social enterprise dedicated to promoting health and wellbeing and reducing health inequalities. Health Exchange provides services, which support and encourage people in making their own positive choices for a healthier lifestyle, and helps them convert those positive choices into actions and results. Working directly with individuals, and through local community and faith organisations, GP’s, primary care centres, health centres, pharmacies, hospitals and local authorities Health Exchange encourages people to take control of their health and wellbeing.
