QIS2012 - The Standard for Health Management Programmes and Interventions is the new Standard developed by QISMET which will be universally applicable and therefore offers providers of newer programmes the opportunity to apply for certification
QISMET is to get up to £335,000 to test and develop innovative approaches to improve health and wellbeing, Care Services Minister Paul Burstow announced.

We are a user led charity which means people with arthritis are at the heart of our work - they forms our membership, are involved in all our activities and direct what we do. We also provide self management training services, helping people with many different types of long term health condition to improve their health.

Health Exchange is a social enterprise dedicated to promoting health and wellbeing and reducing health inequalities. Health Exchange provides services, which support and encourage people in making their own positive choices for a healthier lifestyle, and helps them convert those positive choices into actions and results. Working directly with individuals, and through local community and faith organisations, GP’s, primary care centres, health centres, pharmacies, hospitals and local authorities Health Exchange encourages people to take control of their health and wellbeing.
The Living with Long-term Health Conditions Programme is one of the services Coventry Community Health Services provides to 1,000 adults and children across Coventry each day, delivered from community clinics, GP practices, and in people's own homes.
